Transitions is a volunteer based program providing case management, respite, companionship, light housekeeping and meal prep, transportation and someone to help with errands.
Hospice care brings hope to people by helping them live life as fully as possible when there is no cure. Hospice care involves a team approach to providing quality end of life care. The goal is to lessen symptoms and improve quality of life. This is achieved through expert medical care, pain and symptom management, and emotional and spiritual support expressly tailored to the patient’s needs and wishes.
Bereavement services are offered to the community through a variety of support groups and individual counseling.
Transitions is for persons who have a life limiting condition with a prognosis of approximately 2 years or less. These clients may be seeking curative treatment.
Hospice care is for any person who has a terminal illness. Hospice is appropriate when there is a life expectancy of six months or less and when the patient is no longer seeking a cure.
Bereavement Services are available to anyone in the community who has experienced the death of a loved one, regardless of the circumstances.
FEES: While there are NO FEES for our pre-hospice Transitions Program and our Bereavement Services, there is reimbursement made available for Hospice Care through Medicare, Medicaid and most Private Insurance Companies. No one is ever turned away for inability to pay.
